Our Service Dog Travel Information Packet is a combination of products to assist you when traveling with your service animal. The Travel Packet includes:

  1. Our Embroidered Service Dog ID Pocket Bag
  2. Laminated USA Federal ADA access card
  3. A two sided metal ID tag
  4. A red engraved service dog tag with ADA & Aviation Protection phone numbers
  5. A Medical Information card.
The Service Dog ID Bag comes in 13 different colors and is convenient for carrying all of your paper work. It has two zipper compartments. One of the compartments has a clear vinyl window so your information can be easily read. The clip on top of the bag enables you to attach it to your luggage or a vest.

The laminated USA Federal ADA access card and two sided metal ID tag come with a snap clip lanyard. We will put your dog's information on one side of the ID and your information on the other side.

A 2.5" shield style, plastic tag (engraved on both sides) can be clipped to the dog's collar, harness, or vest. One side of the engraved tag states that this is a service dog traveling with a disabled handler. The other side is the ADA law and Aviation Protection information with phone numbers.

The Medical Information Card has the medical information that you require on it and has a place for your doctor's signature verifying that you require the assistance of a service animal

Convenient method of carrying all necessary information when traveling with your service animal.
